POSITION SUMMARY

The Housing First ICM Team Leader oversees the daily operations and development of clinical operations, leading an inclusive, collaborative, learning-oriented transdisciplinary team that is collectively committed to the program model, values and the best possible service for the client. Reporting to the Director, Clinical Services (DCS), the ICM Team Leader develops, implements, monitors, and evaluates the clinical program, in alignment with RainCity principles, policies and practices. Working with the DCS, they participate in the hiring, dismissal, discipline, development, and performance evaluation of ICM staff.

The Housing First ICM Team Leader plays a lead role in the program implementation informed by RainCity principles, policies and organization-wide procedures, as well as the program framework; and is responsible for continuous improvement.

The ICM Team Leader promotes the interests of the client population and the program in the community and ensures the program is appropriately integrated with other systems of care. In addition to their operational role, the ICM Team Leader also provides direct counseling services to clients.

The Housing First ICM Team Leader is part of the RainCity excluded management team and works closely with ICM staff, Program Managers, Building/Facility Operations, designated Human Resources Business Partners and Administrative staff.

PROGRAM SUMMARY
Maple Ridge ICM -Intensive case management plays a vital role in the continuum of services and supports for people with serious mental illness and substance use issues in BC.
The ICM Team assists clients by helping them to identify and achieve personalized recovery goals and by assisting in the coordination of support services(e.g. mental health, criminal justice, family, and addictions services).
The ICM team also provides housing opportunities for clients using a ‘housing first’ scattered site approach.
Clients are offered immediate access to rent supplements so they have the opportunity to live in the same kinds of permanent independent apartments that are typically available to people without mental illness, and the rent supplements are provided without requiring participation in psychiatric or substance use treatment.
Self-determination and choice are at the centre of all considerations with respect to the provision of housing and support services.
POSITION QUALIFICATIONS
1. A Bachelor’s level degree in healthcare, health services administration or a related field
2. Minimum of 7 years of experience with programs that deliver direct service to individuals living with mental illness, substance use and physical health issues; 2 years of this experience should include supervising or managing programs, including experience in budgeting, program planning, policy development, and labour relations
3. Active registration with a regulatory college under the Health Professions Act (HPA)
4. Current First Aid and CPR certification
5. A clear vulnerable sector criminal reference check
6. Class 5 driver’s license
7. Advanced knowledge in:
  • Practices for populations with complex needs related to mental health issues, substance abuse, and homelessness
  • Mental health diagnoses, medications, treatments, support resources, and strengths-based recovery model
  • Related physical health issues (e.g. HIV and Hepatitis C),including diagnoses, treatment, and support resources
  • Housing continuum, including market, subsidized and supported systems, as well as program models for special needs populations
  • Oppression and marginalization, specifically as related to aboriginal people and people living with mental health and/or substance abuse issues
  • B.C. health care and social services system

REQUIRED SKILLS, ABILITIES & KNOWLEDGE

PROGRAM SUMMARY 

Assertive Community Treatment (ACT) is an evidence-based transdisciplinary intensive mental health team where 80% of the work occur in the community. Support is provided by ACT clinician’s seven days a week and staff are closely involved in hospital admissions and discharges. The RainCity ACT team also provides housing opportunities for clients using a ‘housing first’ scattered site approach. Clients are offered rent supplements so they have the opportunity to live in the same kinds of permanent independent apartments that are typically available to people without mental illness, and the rent supplements are provided without requiring participation in psychiatric or substance use treatment. Self-determination, choice and harm reduction are at the centre of all considerations with respect to the provision of housing and support services. 

POSITION SUMMARY 

ACT nurses assess, identifies, plans, implements and evaluates the care required to assist ACT team clients in meeting their physical, social, spiritual and psychological needs. The Nurse works within the team’s interdisciplinary clinical framework to provide client centered and strengths based care in accordance with the CRNBC/CRPNBC Standards of Practice and consistent with the mission, vision and values and established policy and protocols of RainCity Housing and the ACT team.  

Many of our clients are certified under the Mental Health Act and knowledge of the Act and its implications and limitations is essential to working as an ACT nurse. 

The Nurse provides treatment services that are both inclusionary and flexible for individuals with mental illness, addictions and/or chronic medical conditions and ensures integration and continuity across the care continuum. The Nurse establishes and maintains relationships with, and consults with community health centers, hospitals, housing providers and the interdisciplinary teams to provide appropriate treatment services based on best practices and client need. The Nurse performs duties such as, intake, mental and physical health and addictions assessment, treatment planning, crisis intervention, consultation with hospitals around care planning and discharge, wound care and medication administration.  

QUALIFICATIONS

Education and Experience
– Baccalaureate in Nursing or Health Sciences (Psychiatric Nursing).
– Current practicing registration with the College of Registered Nurses of British Columbia (CRNBC) or the College of Registered Psychiatric Nurses of British Columbia (CRPNBC).
– Three (3) years’ recent related experience providing treatment services to individuals with mental health, addiction and chronic medical concerns,
– Or an equivalent combination of education, training and experience.
– Valid class 5 BC Drivers License and acceptable drivers abstract. Will require the use of a personal vehicle.

Knowledge and Abilities
Broad knowledge of nursing theory and practice within a client and family centred model
Broad knowledge of diagnoses, treatment and support resources in the areas of mental illness, substance use, physical health, including HIV and Hepatitis C.
Broad knowledge of the principles of recovery in mental illness and problematic substance use.
Knowledge of harm reduction principles and activities
Basic knowledge of legislation and government policies and procedures that relate to client, including the Residential Tenancy ACT and the BC Mental Health Act.
Demonstrated understanding of oppression and marginalization, specifically as related to people with experience of a mental illness, addiction, or homelessness.
Ability to work with individuals from diverse backgrounds in a downtown environment and be flexible and creative in providing services
Ability to communicate effectively, both orally and in writing.
Ability to work both independently and collaboratively as a member of a multidisciplinary team.
Ability to operate related equipment.
Physical ability to perform the duties of the position.

Housing First is a recovery-oriented supportive housing approach that operates on the principle that homeless people living with mental illness should be offered immediate access to rent supplements so they can live in the same kinds of permanent independent apartments that are typically available to people without mental illness, and that housing should be provided without requiring participation in psychiatric or substance use treatment. POSITION SUMMARY: the Community Mental Health Clinician (CMHC) works within the context of a client centered care model, in accordance with the standards of professional practice and consistent with the mission, vision and values and established policy and protocols of RainCity Housing and the ACT team. The CMHC works within the trans-disciplinary ACT team and as part of a broader integrated system to deliver services to clients experiencing various levels of substance misuse and mental illness. Establishes and maintains non-judgmental therapeutic relationships with clients, collaborates and consults with colleagues and other community resources, and assists with developing/providing education services as required.

The CMHC also performs Service Coordination duties for a group of approximately 10 ACT participants, maintaining oversight to ensure that client’s self-identified goals for recovery and community integration are supported by appropriate services and practices, consistent with the values of self-determination/choice and harm reduction. Working in collaboration with the other members of the ACT team, the CMHC coordinates the provision of ACT services for clients served by the team members in their area of specialization. Support services are often provided in non-traditional environments, such as the participant’s home, coffee shops, and community centres. As ACT follows the Housing First model, supporting participants through evictions and moves is also a core component of the work, ensuring that learnings and skills from each housing situation are not lost.

Qualifications: many forms of education and experience are valued, including volunteering and lived experience of mental illness / addiction/ homelessness; Minimum education and experience includes:

  • Master’s degree in an Allied Health Discipline from a recognized program and registration with a professional body (e.g., Masters level Registered Social Worker, Registered Clinical Counsellor, Canadian Certified Counsellor, Occupational Therapist)
  • Three (3) years’ recent related experience providing professional services to individuals who have substance abuse and mental health illness, including two (2) years post graduate experience, with urban homeless populations with serious mental illness, substance addiction and complex health conditions.
  • Current first aid and CPR
  • Valid B.C. Driver’s License and acceptable drivers abstract. Use of a personal vehicle is required

Knowledge and Abilities

  • Demonstrated understanding of oppression and marginalization, specifically as related to people with experiences of a mental illness, addiction, or homelessness.

The Occupational Therapist (OT) applies Occupational Therapy concepts and practices to assess the client’s current functional status and develop a rehabilitation treatment plan. The OT works with clients, members of the ACT team and external service providers to ensure a unified, multidisciplinary approach to treatment. In collaboration with the Peer Specialist, co-leads the ACT Team’s Wellness Management Recovery Program via developing and facilitating group sessions, providing 1:1 services and providing in-service and consultation support to ACT team Members.

PROGRAM SUMMARY 

Assertive Community Treatment (ACT) is an evidence-based transdisciplinary intensive mental health team where 80% of the work occur in the community. Support is provided by ACT clinician’s seven days a week and staff are closely involved in hospital admissions and discharges. The RainCity ACT team also provides housing opportunities for clients using a ‘housing first’ scattered site approach. Clients are offered rent supplements so they have the opportunity to live in the same kinds of permanent independent apartments that are typically available to people without mental illness, and the rent supplements are provided without requiring participation in psychiatric or substance use treatment. Self-determination, choice and harm reduction are at the centre of all considerations with respect to the provision of housing and support services. 

POSITION SUMMARY 

ACT nurses assess, identifies, plans, implements and evaluates the care required to assist ACT team clients in meeting their physical, social, spiritual and psychological needs. The Nurse works within the team’s interdisciplinary clinical framework to provide client centered and strengths based care in accordance with the CRNBC/CRPNBC Standards of Practice and consistent with the mission, vision and values and established policy and protocols of RainCity Housing and the ACT team.  

Many of our clients are certified under the Mental Health Act and knowledge of the Act and its implications and limitations is essential to working as an ACT nurse. 

The Nurse provides treatment services that are both inclusionary and flexible for individuals with mental illness, addictions and/or chronic medical conditions and ensures integration and continuity across the care continuum. The Nurse establishes and maintains relationships with, and consults with community health centers, hospitals, housing providers and the interdisciplinary teams to provide appropriate treatment services based on best practices and client need. The Nurse performs duties such as, intake, mental and physical health and addictions assessment, treatment planning, crisis intervention, consultation with hospitals around care planning and discharge, wound care and medication administration.  

QUALIFICATIONS

Education and Experience
– Baccalaureate in Nursing or Health Sciences (Psychiatric Nursing).
– Current practicing registration with the College of Registered Nurses of British Columbia (CRNBC) or the College of Registered Psychiatric Nurses of British Columbia (CRPNBC).
– Three (3) years’ recent related experience providing treatment services to individuals with mental health, addiction and chronic medical concerns,

The Lux provides 92 units of supported housing to people who are coping with the realities of homelessness, substance use, and complex physical and mental health concerns. The Lux is staffed 24/7 by support workers, the building is monitored 24/7 by front desk staff at the main entrance.

ABOUT THE ROLE:

Reporting to the Management Team or designate, the Personal Care Aide is responsible for providing assistance, support, and basic skill training to tenants in their living environment while encouraging independence, responsibility, and healthy lifestyle choices.  They will also have a focus on medication support, hygiene assistance, housekeeping, harm reduction and coordination of community resources. The Personal Care Aide will observe and document the program participants health condition and environment, reporting and addressing unsafe conditions, and provide support and guidance.

Qualifications:

Many forms of education and experience are valued, including volunteering and lived experience of mental illness / addiction/ homelessness; Minimum education and experience includes:

  • Health Care Attendant Certificate, including registration in good standing with the BC Care Aide & Community Health Worker Registry, or equivalent education/experience
  • A minimum of two (2) years post-secondary education in mental health & addictions and a minimum of two (2) years’ experience in a similar setting with a mental health and addictions related agency, with responsibility for front line service delivery, crisis intervention, debriefing and support, conflict resolution and case planning or an equivalent combination of education, training, and experience.
  • Extensive working knowledge of psychosocial rehabilitation approaches and services including individual service plan development and implementation and life skills training
  • A solid background and knowledge of concurrent disorders with related work experience
