SALARY: $26.42/hour (BCGEU grid 34) + $2.50/hour as a premium for night shifts.
TYPE: Temporary until November 30, 2022, Part-time, 0.9 FT
SCHEDULE: This position requires the person to work a combination of day and evening shifts and to work a share of weekends. ACT team members work on a rotation. Schedules are subject to change to meet operational requirements and client needs. Includes 30 minute unpaid meal break

Job requires employee to be able to drive for outreach visits, have a valid drivers license with a clear abstract in order to have a MODO account.

the Outreach Worker provides a range of service coordination and case management services for chronically homeless individuals living in the community, who are transitioning from homeless living into apartment living and who have concurrent mental health and substance use diagnoses. Using a strengths-based model the Outreach Worker collaborates with clients and the ACT team in the development of a comprehensive recovery plan that is reviewed and modified with the client on a regular basis, ensuring that the client’s self-identified goals are supported by the ACT team and other relevant services. The Outreach Worker works closely and collaboratively with other ACT staff, family and external service providers, advocating for access and rights as needed. They also provide supports including medication administration, crisis intervention, budgeting and assistance with tenancy issues.
The Outreach Worker also performs Service Coordination duties for a group of approximately 10 ACT clients, maintaining oversight to ensure that client’s self-identified goals for recovery and community integration are supported by appropriate services and practices, consistent with the values of self-determination/choice and harm reduction. Working in collaboration with the other members of the ACT team, the OW coordinates the provision of ACT services for clients served by the team members in their area of specialization. They also provide medication administration, crisis intervention, budgeting and assistance with tenancy issues. As ACT follows the Housing First model, supporting participants through evictions and moves is also a core component of the work, ensuring that learnings and skills from each housing placement are not lost. This work is intensive and highly demanding, occurring primarily in community settings, such as the person’s home, grocery stores, coffee shops, and community centres.
ACT is an evidence-based transdisciplinary mental health team that has a client-staff ratio of 10:1, with 80% of the work occurring in the community. Crisis support is available seven days a week, 24 hours a day. Program staffs are closely involved in hospital admissions and discharges. The RainCity ACT team also provides housing opportunities for clients using a ‘housing first’ scattered site approach. Clients are offered immediate access to rent supplements so they have the opportunity to live in the same kinds of permanent independent apartments that are typically available to people without mental illness, and the rent supplements are provided without requiring participation in psychiatric or substance use treatment. Self-determination, choice and harm reduction are at the centre of all considerations with respect to the provision of housing and support services.
many forms of education and experience are valued, including volunteering and lived experience of mental illness / addiction/ homelessness; Minimum education and experience includes:
· Diploma in community social services
· Two years of recent, related work experience with an emphasis on mental health, addictions,
homelessness and/or social services; including one year of service planning experience.
· Or an equivalent combination of education, training and experience
· Current First Aid and CPR certification.
· Class 5 driver’s license and access to a vehicle. Allowance paid for use of personal vehicle.
Knowledge and Abilities
· Knowledge of principles of community integration for people living with a mental illness.
· Knowledge of the principles of recovery in mental illness and problematic substance use.
· Knowledge of harm reduction principles and activities
· Basic knowledge of diagnoses, treatment and support resources in the areas of mental illness, substance use, physical health, including HIV and Hepatitis C.
· Basic knowledge of psychopharmacology
· Basic knowledge of counseling skills, including motivational interviewing principles.
· Basic knowledge of other health care disciplines and their role in client care.
· Basic knowledge of legislation and government policies and procedures that relate to client, including the Residential Tenancy ACT and the BC Mental Health Act.
· Demonstrated understanding of oppression and marginalization, specifically as related to people with experience of a mental illness, addiction, or homelessness.
· Ability to work with individuals from diverse backgrounds in a downtown environment and be flexible and creative in providing services
· Ability to identify and work to increase client strengths, including the use of hope inducing strategies and assisting the client in self-managing their illness and other aspects of their life.
· Ability to commit to service partnerships and build relationships by providing assistance and support.
· Ability to effectively and safely resolve crisis situations
· Ability to apply knowledge of theory and practice to a case management process.
· Ability to communicate effectively, both orally and in writing.
