PROGRAM DESCRIPTION

The Mobile Peer Team in Maple Ridge/Pitt Meadows consists of peers and a RainCity program manager.  The team works in collaboration with the existing services in Maple Ridge to provide support for people who use substances. The aim is to connect with individuals who are underserved or hard to reach and/or reluctant to access counselling, to the appropriate supports and services that exist in the community.  The peer team incorporating their community knowledge, current best practices and honouring cultural practices developed the services provided.  The team currently provides information, education, overdose prevention resources and harm reduction supplies.

The Outreach Social Worker- Community Counselling position is an expansion of the existing Mobile Peer Team.  This expansion of the Mobile Peer Team strengthens the capacity of the peer team to provide peer based counselling and increases access to sustainable community-based counselling opportunities. The program will provide training and supervision to empower team members to provide peer-counselling services. This increases access to services for individuals who are not engaging with mainstream services and reduces wait lists.

Peer support is the inclusion of people with lived experience of trauma and adversity in solving problems experienced by people struggling with similar challenges. It is the practice of using ones own knowledge of recovery and lived experience to promote others recovery. By including a diverse group of people that reflect our client population, we are able to support community services encompassing the pathways to healing and recovery. Our peer teams not only offer hope and support, they are able to share their own stories of service navigation and support individuals on their own personal journeys to wellness.

JOB SUMMARY

Reporting to the Manager of Harm Reduction Support Services or designate, the Social Worker-Community Counselling works within the context of a client centered care model, in accordance with the standards of professional practice and consistent with the mission, vision and values and established policy and protocols of RainCity Housing and the BCCSW. The SWCC works with the Peer Mobile Outreach team and as part of a broader integrated system to deliver services to clients experiencing various levels of marginalization related to substance use and mental health. THE SWCC provides short term individual or group counselling services to marginalized clients identified by the peer team and/or existing community services who are not accessing or reluctant to access existing community services or available supports.  The SWCC provides services to marginalized individuals who would not otherwise be able to seek these services; provides appropriate treatment/support including individual and group therapy and education; and identifies the client’s motivation and their stage in the treatment process. The SWCC encourages and supports connection with existing community resources and engagement to support treatment. Establishes and maintains non-judgmental therapeutic relationships with clients, collaborates and consults with colleagues and other community resources, assists with developing/providing education services as required, and participates in program sponsored interdisciplinary research and/or quality assurance activities.

DUTIES

  1. Provide individual counseling to clients regarding substance use, trauma, family and personal relationships.
  2. Provide group counseling to clients in areas related to substance use, including harm reduction, recovery and relapse prevention, trauma, family.
  3. Provide strengths based therapy using motivational interviewing techniques, narrative approach and trauma informed practices.
  4. Counsel and educate clients in harm reduction concepts and practices of mindfulness
  5. Provide direct interventions for management of crisis and life threatening situations.
  6. Consult with external service providers and facilitate access to other health professionals to ensure a unified, multidisciplinary approach to treatment.
  7. Engage and deliver training on harm reduction, motivational interviewing, understanding problematic substance use and recovery, integrated treatment, and relapse prevention; support cross-disciplinary learning by consulting, debriefing and modeling to the peer team members.
  8. Establish a dignifying and purposeful relationship with the client, contacting and getting to know the client as a unique individual.
  9. Assist the client in self-managing all aspects of their life, including mental illness, substance use and other health conditions by such methods as information and education, coaching, practical problem-solving, and linkages with peer supports, and consumer advocacy groups.
  10. Coordinate and monitor referrals to community services and advocate for client rights re: dignity, access, self-determination, privacy, access to information; as required, initiate and/or participate in case conferences that include the client, family, other informal supports and service providers to ensure coordination of care and resolution of emergent issues.
  11. Provide supports for crisis intervention and prevention by identifying potential crises and implementing preventative plans, responding to crises as they arise (e.g. mental health, physical health, housing), report all crises to the Manager of Harm Reduction Services, using critical incident forms as required.
  12. Actively participate in building a skilled, cohesive, collaborative staff team, by methods such as demonstrating commitment to program values and principles, resolving conflict and negativity as they arise, assisting in training and mentoring of the peer team.
  13. Engage in on-going program planning, development, evaluation and quality improvement activities that facilitate team goal setting, problem solving conflict resolution and collaboration by methods such attending meetings and reviewing team policies, procedures, and communications systems.
  14. Prepare and maintain written and computerized records, compile reports and complete other program documentation, including writing and updating service plans for primary caseload, writing progress notes, completing incident reports, on-call logs, letters, and other documentation.
  15. Perform related work as assigned.

 QUALIFICATIONS

  • Bachelors’ degree in Social Work from a recognized program
  • Registered with BCCSW • Three (3) years recent related experience providing professional services to individuals who have substance abuse and mental health illness, including two (2) years post-graduate experience, with urban homeless populations with serious mental illness, substance addiction and complex health conditions. Experience with motivational interviewing, narrative approach, strength based and trauma informed practices preferred.
  • Current first aid and CPR • Valid B.C. Driver’s Licence and an Acceptable Drivers Abstract. Use of a Personal Vehicle is Required.

KEY SKILLS & ABILITIES – for supporting people using our services

  • Demonstrated understanding of oppression and marginalization, specifically as related to people with experience of a mental illness, addiction, or homelessness.
  • Comprehensive knowledge of psychotherapeutic and counseling skills, including motivational interviewing, cognitive behavioural and narrative therapy
  • Broad knowledge of bio-psycho-social issues that may occur in response to changing mental health and alcohol and/or other drug needs.
  • Broad knowledge of mental health and substance use diagnoses and their interactions.
  • Broad knowledge of DSM IV diagnostic classification system.
  • Knowledge of the principles of recovery, and recovery models and practice in mental illness and problematic substance use.
  • Knowledge of harm reduction principles and activities.
  • Basic knowledge of other health care disciplines and their role in client care.
  • Basic knowledge of legislation and government policies and procedures that relate to client, including the BC Mental Health Act.
  • Ability to work with individuals from diverse backgrounds and be flexible and creative in providing services
  • Ability to identify and work to increase client strengths, including the use of hope inducing strategies and assisting the client in self-managing their illness and other aspects of their life.
  • Ability to commit to service partnerships and build relationships by providing assistance and support.
  • Ability to effectively and safely resolve crisis situations.
  • Ability to apply knowledge of theory and practice to a case management process.
  • Ability to communicate effectively, both orally and in writing.
  • Listening and information seeking skills that promotes communication and lead to a co-operative approach to problematic client actions and choices; problem solving within a multidisciplinary setting.
