concurrent disorders outreach team
The Concurrent Disorders Outreach (CDO) Team works in the community with folks that have a long history of homelessness, many of whom are living with mental illness, addiction, and other challenges. They work with people as long as they need their services, until they have the same kind of supports in place where they live, have grown more independent, or have moved into a new neighbourhood.
The outreach team works collaboratively with all referral agencies and service providers to ensure clients receive broad community support.
Eligible referrals include people who:
- Are experiencing homelessness/high levels of repeat shelter use.
- May have a formal mental health diagnosis, or, if their mental health status is unclear, a mental illness will be strongly indicated.
- Use substances.
- Reside in the Downtown Eastside.
- Will have complex needs that require intensive outreach interventions.
Referrals can be made by phoning (604) 254-3727. The team recognizes that the solution for homeless individuals with concurrent disorders is a coordinated and comprehensive response from the community, and they work closely and collaboratively with all referral agencies and service providers. The RainCity Outreach Team is a partnership project funded jointly by Vancouver Coastal Health, Service Canada and BC Housing.

The Homeless Outreach Program (HOP) works directly with people sleeping in public spaces and assists them in linking to appropriate services and housing, and accompanying them to appointments.
The team’s goal is to help break the cycle of homelessness, instability and poor health experienced by people living outdoors.
